Instruction for IE:
- Click on the download link above
- Save the form on you PC by clicking the button “Save file as PDF” in the bottom right corner of the form
- Open the form on your PC and fill in all the fields
- Send the form by e-mail to: arrivals.hanoytangen@semcomaritime.com (if you use MS Outlook you can click the e-mail address in the top of the form)
- Make sure it says “HT-arr-form” in the subject field of the e-mail
- Please remember to attach: Crew list and list of last ten ports visited
